In order to admit your teenager into Turning Winds, we will need you to complete the confidential, HIPAA compliant form below. Once this form has been completed, our amazing clinical and medical team will review the information submitted to ensure that your child qualifies for enrollment into our treatment center. We appreciate your time in completing the application and look forward to hopefully having your teen join the Turning Winds Family.
